READ BEFORE REGISTERING: In consideration of being allowed to participate in any way in an ON-X Naturals Sanctioned Event in partnership with the Drug Free Athletes Coalition (DFAC), related events and activities, the Athlete acknowledges, appreciates, and agrees that: The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; the Athlete KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, BOTH KNOWN AND UNKNOWN, EVEN IF ARISING FROM THE NEGLIGENCE OF THE ON-X Naturals Presidents, Owners, Directors, staff or others, and assume full responsibility for Athlete participation; and, these risks and dangers may be caused by Athlete's own actions or inaction’s, the actions or inactions of others participating in the activity, the condition in which the activity takes place, or the negligence of the “releases” named below; Athlete willingly agree to comply with the stated and customary terms and conditions for participation. If, however, the Athlete observe any unusual significant hazard during Athlete's presence or participation, the Athlete will remove themselves from participation and bring such to the attention of the nearest official immediately; and, There may be other risk and social and economic losses either not known to the Athlete or not readily foreseeable at this time; and the Athlete fully accept and assume all such risks and all responsibility for losses, costs, and damages the Athlete incurs as a result of Athlete's participation or that of the minor in the activity. The Athlete, for themselves and on behalf of their heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS ON-X Naturals/ON-X Entertainment LLC and the DFAC, and their owners, officers, officials, agents, or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct ON-X Naturals Sanctioned Events WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, OR LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, TO THE FULLEST EXTENT PERMITTED BY LAW. THE ATHLETE HAS READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
 

Furthermore: For good and valuable consideration, the receipt of which is hereby acknowledged, the athlete hereby consents to the photographing of the athlete and the recording of the Athlete's voice and the use of these photographs and/or recordings—including any digital alterations, enhancements, or AI-generated derivatives created from the athlete’s likeness—singularly or in conjunction with other photographs, recordings, or digital media for advertising, publicity, commercial, or other business purposes.

The Athlete understands that the term "photograph" as used herein encompasses still photographs, motion picture footage, and any synthetic or AI-generated imagery derived from the Athlete's physical appearance captured during the event. The Athlete further consents to the reproduction and/or authorization by ON-X Entertainment, LLC/ON-X Naturals and the DFAC, Todd Rogers, James Mickles, and media assigned to ON-X Naturals Sanctioned Events to reproduce and use said photographs, recordings, and AI-enhanced media for use in all domestic and foreign markets.

Further, the Athlete understands that others, with or without the consent of ON-X Entertainment, LLC/ON-X Naturals and the DFAC may use and/or reproduce such photographs and recordings. The Athlete hereby releases ON-X Entertainment, LLC/ON-X Naturals and the DFAC, and their owners, shareholders, companies, directors, officers, agents, employees and customers, and appointed advertising agencies, from all claims of every kind on account of such use. The athlete takes full responsibility of the athlete's actions in respect to rules and regulations governed by ON-X Naturals and all rules of the Venue.
